I have five patients right now that I am working through new onset symptoms to see if any of them are adverse drug effects. It entails stopping one med at a time, giving times for symptoms to abate, and then following up with the patient. Our PCPs do not have time for this, they would simply stop the med and mark it as an allergy. One patient with a diabetes meds has an A1c in the 5s. That means the med would be stopped and abandoned, and we could lose very tight control of the patient's diabetes and never regain it, without actually knowing whether or not the med caused the symptom.Wow. That is really unfortunate.
We now have a pharmacist in the ER at the VA. I tried to get one in Aus and she was just about to start as I left.
I know I make better medication decisions now. And quicker because he has reformatted how we order and standardized the choices. And I've been begging to get phenobarbital for alcohol withdrawal forever. He shows up and gets it through and protocolized.
Working with a clinical pharmacist is like driving a car with anti-lock brakes, a back-up camera, and lane keep assist. Of course, you can do it without, but you are kidding yourself if you think it is as good or as safe.
As far as we know at the moment, that function has been eliminated altogether. Which means patients are going to get the short end of the stick. It makes no sense.That sucks, and should never happen. Are they going to contract out the pharmacy service for the base, or double workload on someone else?
I don't remember ever really being a fan, but the only electric vehicle I wouldn't be caught dead in an electric vehicle unless it's on a cart path so I'm not the best example here.
I wish more people would take a step back and ask themselves if they really think billionaires will act in the best interests of the nation or use their influence to do more of what got them there in the first place though.

Completely agree. Only reason I was pointing out cost was because this is all being done in the guise of saving taxpayer money, which it won't do either. There is no positive in these kinds of situations.And patient care will get worse.

IMO, it’s a really bad idea to just “hope” someone is acting altruistically when his history shows he has never acted to benefit anyone other than himself in the past.

I would say it’s trending towards a nefarious….rather than dubious…plan, but yeah.My optimism is dwindling on the subject. Its either a good plan very poorly executed or a dubious plan as you say (still poorly executed)
Well, as suggested, I thought about it.
What I can't decide is whether it's a parable about:
1) Democrats (as Scar) controlling most of the larger cities in America for the past 50 years, primarily by promising African Americans and poor white people (the hyenas) that only Scar cared about them and would fully share with them the full bounty of his reign as King. While the hyenas have not yet turned on Scar en masse, many individual hyenas are finally beginning to question the wisdom of their blind loyalty to him.
OR
2) The 2024 election. After 50++ years of loyalty to Scar (the DNC and/or Biden), many, many hyenas (,black hyenas, Hispanic hyenas, young hyenas, working class male hyenas, to name a few) turned on Scar and ate him (sided with Republicans as a repudiation of the failed policies of the left and their inability to secure a better life for ALL hyenas). While fundamentally unsure of how Simba's (Trump's) reign would ultimately affect them, they knew what they would get with four more years of Scar, and chose the former.
